This contest is referred to as the "Playoff Payoff Tickets Giveaway Contest" (the "Contest"). The contest is co-sponsored by BMO Harris Bank N.A., 111 West Monroe Street, Chicago, Illinois 60603 ("BMO") and Sports Radio Chicago, LLC ("ESPN 1000"). BMO and ESPN 1000 are collectively referred to herein as the "Sponsor".


This Contest is open to legal residents of Illinois, Wisconsin, Indiana, Michigan and Iowa, 18 years of age or older, except any individuals (including but not limited to employees, officers, directors, consultants, independent contractors, and interns) who have, within the past six (6) months, performed services for Sponsor, or any organization responsible for sponsoring, fulfilling, administering, advertising or promoting the Contest on ESPN 1000 or supplying the prize, and/or their respective parent, subsidiary, affiliated and successor companies, and immediate family and household members of such individuals, are not eligible to enter or play. "Immediate family" shall mean parents, step-parents, children, step-children, siblings, step-siblings, or spouses. "Household members" shall mean people who share the same residence at least three months a year.


The Contest begins at 5:00 a.m. CT on Monday, April 22, 2013 and will continue until the earlier of (i) the date on which the Chicago Bulls and the Chicago Blackhawks have been eliminated from the 2013 playoffs, or (ii) the date on which the National Hockey League and the National Basketball Association 2013 playoffs have ended (the "Contest Period"). To clarify (i), above, the Contest Period will not expire upon the elimination of either the Chicago Bulls or the Chicago Blackhawks from the 2013 playoffs. Both teams must be eliminated from the playoffs for the Contest Period to expire under (i) above. Upon the expiration of the Contest Period, no more qualifiers will be accepted and no more prizes will be awarded.

During the Contest Period, participants must tune into the Carmen & Jurko Show (M-F 10A-2P CT) and the Waddle & Silvy Show (M-F 2P-6P CT) for their chance to win Chicago Bulls or Chicago Blackhawk home game tickets. During the 11A CT and 4P CT hour each weekday, a basketball bank shot and/or hockey buzzer sound effect will be broadcasted. When the sound effect is broadcasted, participants must place a call to (312) 332-3776 (the "Hotline"). The tenth (10th) caller will be selected to answer a trivia question about the Chicago Bulls or Chicago Blackhawks. The caller who answers the trivia question correctly will be qualified to win the prize, so long as the caller satisfies the eligibility requirements set forth above. At most, two (2) qualifiers per weekday (one from each show) will be entered into the prize drawing.

Only calls to the specified phone number during the specified time can win. Sponsor is not responsible for inability of caller to complete call within specified time, whether due to busy lines, disconnections, telephone malfunctions or other circumstances. There is no limit on the number of calls that the caller may place to the Hotline in an attempt to win a prize. However, callers should not call the Hotline after the sound effects are broadcasted for promotional purposes, such as, the advertising of this Contest, or a demonstration of the sound effects. Callers cannot qualify to win a prize at these times.

In the event the selected caller does not correctly answer the trivia question that caller will be disqualified and the next caller will be given the opportunity to correctly answer the same trivia question, this will continue for up to four (4) additional callers should all the preceding callers incorrectly answer the trivia question for that round, for a total of five (5) opportunities to correctly answer that round's trivia question. Callers will be banked in the order their call was received during the initial call-in and each of those callers will be given the same single opportunity to correctly answer the same trivia question. In the event that after five (5) callers have been given the opportunity to correctly answer the trivia question and were not able to do so, that round will be closed, no qualifier will be selected and the next opportunity to call in will be announced. Once a trivia question has been asked it will not be reused in the next round nor any later rounds.

Prior to each Chicago Bulls and Chicago Blackhawks home playoff game, one (1) winner will be randomly selected from all qualified entries to receive a prize. Winners will be announced on ESPN 1000 between 10 a.m. and 6 p.m. CT, Monday-Friday, at least two (2) days prior to each Chicago Bulls and Chicago Blackhawks home playoff game. Winner must provide his or her name, age, address and phone number.

Each winner will receive two (2) tickets to an upcoming Chicago Bulls or Chicago Blackhawks playoff home game. Game dates and times to be announced. Tickets will be for seats in the 100 or 300 level (depending on availability) of the United Center. Approximate retail value for a pair of Chicago Bulls tickets are as follows: Round 1 tickets: Approximate retail value: $140 a pair for 300 level; Round 2 tickets: Approximate retail value: $180 a pair for 300 level; Round 3 tickets: Approximate retail value $240 a pair for 300 level; Round 4 tickets: Approximate retail value $400 a pair for 300 level. Approximate retail value for a pair of Chicago Blackhawks tickets are as follows: Round 1 tickets: Approximate retail value: $800 a pair for 100 level and Approximate retail value: $350 a pair for 300 level. Round 2 tickets: Approximate retail value: $1,200 a pair for 100 level and Approximate retail value: $460 a pair for 300 level. Round 3 tickets: Approximate retail value: $1,600 a pair for 100 level and Approximate retail value: $750 a pair for 300 level. Round 4 tickets: Approximate retail value: $2,400 a pair for 100 level and Approximate retail value: $1,600 a pair for 300 level.

In the event a winner is awarded tickets to a game which the Chicago Bulls or the Chicago Blackhawks fail to advance, the winner will not receive an alternative prize. In such case, the approximate retail value of the prize will be $0. The final number of prizes will be determined by the number of playoff games in which the Chicago Bulls and the Chicago Blackhawks are eligible to participate. Odds of winning a prize depend on the number of calls to the Hotline and the number of qualifiers entered into the drawing.

Each winner will be notified of his or her winning entry by phone. Winner must determine whether winner will accept the prize while on the call. Winners may pick up their prize at the security loading dock located at the ABC Company Building located at 190 N. State Street, Chicago, IL 60601 during normal business hours (8 a.m. to 5:30 p.m. CT- Monday-Friday). In the event winner decides not to claim the prize, Sponsor is under no obligation to award the prize to an alternate winner.

Parking, meals, drinks, gratuities, souvenirs, incidental personal expenses and all other expenses not specified herein are the responsibility solely of winner.

Federal, state and local taxes, if any, are the sole responsibility of the winner, who will receive an IRS Form 1099 reflecting the final actual value of any prize valued at $600 or more. Winners will be required to execute an Affidavit of Eligibility and Liability/Publicity Release. Failure to return the Affidavit of Eligibility and Liability/Publicity Release by the deadline indicated at the time the winner receives the documents, the determination that the winner is not eligible to receive the prize, or the winner's refusal or inability to accept the prize, will result in disqualification of the winner and prize forfeiture, and an alternate winner may be declared, time permitting. A U.S. home address (No Post Office boxes allowed) must be provided on the Affidavit of Eligibility and Liability/Publicity Release. Sponsor is not responsible for any change of email address, mailing address and/or telephone number of a winner.

Winners are not entitled to exchange or transfer prizes or to obtain cash or other substitutes, but Sponsor, in its sole discretion, may substitute prizes of equal or greater value. Each winner is responsible for any costs of redemption or travel to redeem the prize, and any liability which might arise from redeeming or seeking to redeem the prize.

Prize is subject to availability. Sponsor is not responsible for cancellation of a playoff game or for other circumstances that make it impractical to fulfill any element of the Contest (such as the unavailability of tickets).


The Sponsor is not responsible for: (a) causes beyond Sponsor's reasonable control that jeopardize the administration, security, fairness, integrity or proper conduct of the Contest; (b) incorrect or inaccurate information provided by winners; (c) any typographical or other error in any printing or advertising related to the Contest, in the administration or execution of the Contest, or in the announcement of the winners; or (d) cheating or fraud by any caller.

Sponsor shall not be liable to a winner for failure to supply the prize or any part thereof, by reason of any acts of God; any action(s), regulation(s), order(s) or request(s) by any governmental or quasi-governmental entity whether or not the action(s), regulations(s), order(s) or request(s) prove(s) to be invalid; equipment failure; terrorist acts; air raid; blackout; act of public enemy; earthquake; war (declared or undeclared); fire; flood; epidemic; explosion; unusually severe weather; hurricane; embargo; labor dispute or strike (whether legal or illegal); labor or material shortage; transportation interruption of any kind; work slow-down; civil disturbance; insurrection; riot; or any other cause beyond Sponsor's sole control.

Each caller agrees that (a) under no circumstances will caller be permitted to obtain awards for, and caller hereby waives all rights to claim, punitive, incidental, consequential or any other damages, other than for actual out-of-pocket expenses; (b) all causes of action arising out of or connected with this Contest or any prize awarded shall be resolved individually, without resort to any form of class action; and (c) any and all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred, excluding attorneys' fees and court costs.


By participating, callers agree to be bound by these Official Contest Rules and the decisions of the Sponsor, which shall be final in all respects. These Official Contest Rules shall govern in the case of any conflict between the Official Contest Rules and any other Contest related materials, or ambiguity related to other Contest related materials. By participating, callers agree that all issues and questions concerning the construction, validity, interpretation and enforceability of these Official Contest Rules, participant's rights and obligations, or the rights and obligations of Sponsor in connection with the Contest, shall be governed by, and construed in accordance with, the laws of the State of Illinois, without giving effect to any choice of law or conflict of law rules. By participating, callers consent to the jurisdiction and venue of the federal, state and local courts located in Cook County, Illinois.

By participating in the Contest, each caller agrees to release Sponsor, Bank of Montreal, Chicago Blackhawk Hockey Team Inc., Chicago Bulls and their respective parent, subsidiary, affiliated and successor companies, advertising and promotion agencies and prize suppliers, and each of their respective officers, directors, agents, representatives and employees, as well as each of their respective successors, representatives and assigns (collectively, the "Released Parties") from any and all actions, claims, injury, (including death), losses or damages arising in any manner, directly or indirectly, from participation in the Contest and acceptance or use of the prize awarded to the caller. Each caller acknowledges that the Released Parties are not in any manner responsible or liable for any warranty, representation or guarantee, expressed or implied, in fact or in law, relative to any prize.

Callers authorize the Released Parties to use their name, voice, likeness, biographical data, city and state of residence and entry materials in programming or promotional material, in any media now known or hereafter created or conceived, worldwide in perpetuity, or on a winners list, if applicable, without further compensation unless prohibited by law. Sponsor is not obligated to use any of the above mentioned information or materials, but may do so and may edit such information or materials, at Sponsor's sole discretion, without further obligation, notice or compensation.


Void where prohibited. All federal, state and local laws and regulations apply. Winners are required to comply with all United Center rules and regulations. Sponsor is not responsible for any printing or typographical error in these Official Contest Rules, the administration of the Contest, or the announcement of the winners. In the event Sponsor is prevented from continuing or completing the Contest, or the integrity or feasibility of the Contest is undermined by any event or cause not reasonably within Sponsor's control, including but not limited to, electronic disruption, virus, bugs, tampering, printing or production errors, or other causes beyond Sponsor's control that disrupt the administration, security or proper play of this Contest, Sponsor shall have the right, in its sole discretion, to abbreviate, modify, suspend, cancel or terminate the Contest without further obligation.

Limit one (1) prize per winner, per household for this Contest. Callers who have won any other prizes from ESPN 1000 within sixty (60) days of the beginning of the Contest Period are not eligible to participate in this Contest.

If Sponsor, in its discretion, elects to abbreviate the Contest, Sponsor reserves the right, but not the obligation, to award the prize from among all valid and eligible entries received up to the time of such event or cause.

Sponsor reserves the right, in its sole discretion, to disqualify any individual who tampers with the entry process and to cancel, terminate, modify and/or suspend the Contest in the event of tampering or other circumstances, including but not limited to acts of God or civil disturbance that render it impractical or impossible to complete or fulfill the Contest as originally planned.

The invalidity or unenforceability of any provision of these Official Contest Rules shall not affect the validity or enforceability of any other provision. In the event any provision is determined to be invalid or otherwise unenforceable or illegal, these Official Contest Rules shall otherwise remain in effect and shall be construed in accordance with their terms as if the invalid or illegal provision were not contained herein.

All personal information collected from the Contest will be subject to the terms of the Sponsor's privacy policies which may be viewed at www.bmoharris.com and www.espnchicago.com.
